Payment Restructuring; Care Coordination Payments
As of January, 2010, all health plans in Minnesota are required to have health care homes a part of their provider networks. Health plans have been required, since July, 2010 to pay a coordination fee for members who have enrolled in certified health care homes. Health plans are also required to develop payment terms and terms for the coordination fee for participants of the health care home model.
