Payment restructuring; care coordination payments
The Commissioner of Human Services must establish a payment system that gives per person coordination payments to certified health care homes for providing care coordination services or having an on site employee that coordinates care. The care coordination payments are in addition to quality incentive payments that may be made pursuant to M.S.A. §256B.0754. The care coordination payments are based on levels of care complexity, with the highest fees paid for care given to an individual that requires the most intensive care coordination. In establishing benchmarks or levels of care complexity the additional time and resources needed for patients with limited English skills, cultural differences or other barriers should also be considered. The care coordination payments were to be implemented by of either July 1, 2010 or upon federal approval.
