Provider cost disclosure
Each pharmacy must provide the following information to a patient upon request:
- The pharmacy’s usual and customary price for a prescription drug;
- A record of co-payments and cost-sharing amounts paid to the pharmacy by the patient for up to 2 years;
- The total amount of co-payments and cost-sharing amounts paid to the pharmacy by the patient for the last 2 years.
This information must be readily available to the patient at no cost.
