Requires the vital statistics unit to create a form for reporting abortions. Prohibits the Unit from collecting information that would identify the person who received the abortion. Requires the physician attending an abortion procedure to submit the completed form within 15 days of the close of a reporting month. Requires the Department of Health and Welfare to maintain a compilation of reports as public record.
