All facilities that provide outpatient medical services must maintain medical records for each patient. All records must be legible and accurate, and must include the following information:
- Patient identification;
- Medical history;
- Allergies and abnormal drug reactions;
- Discharge diagnoses;
- Patient consent forms, if applicable;
- Assessment of health status and health care needs;
- Reports of physical examinations;
- Nursing notes; and
- Medication charts.
