Patient Care Plan
Requires general acute hospitals to establish a patient care plan within 24 hours of the patient’s admission. Requires hospitals to keep the care plan current and to ensure that all appropriate personnel have knowledge of the care plan. Requires hospitals to initiate discharge planning upon a patient’s admission and to provide patients and their family members with instructions upon discharge. Hospitals must transfer necessary information when referring or transferring patients to other health care providers for follow up or ancillary care.
