Patient care guidelines
 
Establishes patient care guidelines for home health care pharmacy services. Requires the designation of a primary care physician as responsible for a patient’s care and requires the patient, physician, and pharmacist to understand their responsibilities regarding provision of care and payment for services. Requires the pharmacist to participate in a patient’s drug therapy training. The training must occur and be documented in the patient’s record before a pharmacist may dispense supplies. Pharmacists must reassess the patient’s sufficiency at least once every six months and visit the patient at least monthly; both encounters must be documented in the patient’s record. Requires the pharmacist to have access to patient lab and clinical data.
