Coordinated Care through a Health Home
The Commissioner of Human Services must provide health care homes for beneficiaries of medical assistance who have multiple chronic conditions. The individual must be enrolled in Minnesotacare, the state’s Medicaid program, and have at least one chronic condition. Health home services include complete care management, care coordination, and the use of health information technology. The Commissioner must also establish health teams to support the patient centered medical home, and provide services to eligible individuals. The Commissioner is also authorized to make payments to the health home and the health teams for services rendered.
