Permits individuals to request that the Department of Health and Welfare amend their records. Requires the Department to issue a written determination within 10 days of the request. Clarifies that records created by a third party are not subject to this amendment process. Permits individuals to provide a written response if the Department rejects their amendment request. Allows individuals to request that records of their amendment request and denial be provided whenever the Department discloses their information.
