“Incident reporting system under the Agency for Health Care Administration regulations”
Each health care facility must establish an incident reporting system and written procedures that must be disseminated to all employees. All adverse incidents must be reported to the risk manager within 3 days of occurrence. The risk manager is responsible for the regular and systematic review of all incident reports.
