Discharge and Transfer
Requires nursing facility administrators to notify the Department of Human Services when a Medicaid member requests a transfer or discharge and for the administrator, facility staff, and member to develop a a plan for their transfer or discharge. Requires facilities to transfer records to the member’s new facility and specifies the information that must be included in these records (e.g., physician orders, nursing care plan, diagnosis, etc.). Requires facilities to submit a Case Activity Report to the Department upon receiving a new resident, discharging a resident, or changing a resident’s level or care.
