All intermediate care facilities must maintain patient medical records, which include the following information:
- The patient’s full name;
- The patient’s date of birth;
- The patient’s recent photograph, among other personal information;
- A medical history;
- Age at onset of disability;
- Medication history; and
- Reports of all treatments.
