Records of disclosure
The physician or psychiatric advanced registered nurse practitioner in charge of a patient or the head of the facility caring for the patient must document the disclosure of the patient’s information in their medical record. Such documentation must include the circumstances surrounding the disclosure, the date of disclosure, the identity of the persons that obtained the information, and the specific information that was disclosed.
