Certificates Generally
Vital statistics certificates must contain, at minimum, social security numbers and other items recommended by the federal government.
The state board of health may require certificates to contain additional birth information if such information is necessary for statistical purposes. This statistical birth information is confidential, but individuals may access the information pursuant to a court order or if they seek their own birth record. However, an individual that accesses their own birth record may not access information pertaining to their mother or father.
Certificates must be filed in compliance with the form required by the state registrar and must contain all the requisite data.
