Medical record documentation requirements
The Washington Death with Dignity Act requires physicians to document or insert the following items in their patient’s medical record:
- All requests by a patient for medication to end his or her life in a humane and dignified manner.
- The physician's diagnosis and prognosis, and determination that the patient is competent, and has made an informed decision.
- A report of the outcome and determinations made during counseling.
- The physician's offer to the patient to rescind his or her request.
- A note by the attending physician indicating that all requirements under this chapter have been met.
