Maintenance of Patient Records by Chiropractors
Any chiropractor who treats patients must maintain all treatment records regarding patients treated. The record includes (but is not limited to) X rays, treatment plans, patient charts, patient histories, correspondence, financial data, and billing. The records must be retained by the chiropractor for 5 years in an orderly, accessible file, and must be readily available for inspection by the commission. Patients that request a copy of the record may be charged a reasonable fee to cover mailing and clerical costs. In response to a request for copies by the patient, the chiropractor has 15 days to honor the request.
