Content of Notifications for Notifiable Diseases

For each condition or disease that requires notification to health officials, health care providers must include the following information for each case:

•         Name
•         Address
•         Telephone number
•         Date of birth
•         Sex
•         Diagnosis of disease or condition
•         Pertinent laboratory data
•         Name, address and telephone number of the principal provider
•         Name, address and telephone number of the person providing the report
•         Other information that the Department may require
