Content of Notifications for Notifiable Diseases
For each condition or disease that requires notification to health officials, health care providers must include the following information for each case:
- Name
- Address
- Telephone number
- Date of birth
- Sex
- Diagnosis of disease or condition
- Pertinent laboratory data
- Name, address and telephone number of the principal provider
- Name, address and telephone number of the person providing the report
- Other information that the Department may require
