Voluntary patients — Legal competency — Record
The superintendent of a state mental health hospital must submit a record to the department of social and health services whenever the hospital admits a “voluntary patient.” The record must include the patient’s “name, address, sex, date of birth, place of birth, occupation, social security number, date of admission, name of nearest relative, and such other information as the department may from time to time require.”
