Each licensed ambulatory surgical center and licensed outpatient diagnostic center is required to report the following data elements for each discharge regardless of payer:
1. Patient identifying information;
2. Patient contact information;
3. Patient demographic information, including age, sex, race and ethnicity;
4. Patient’s insurance or third party payer information;
5. Patient’s diagnosis or treatment information.
