Physicians must complete a report on each abortion performed including:
- Date of abortion;
- Name and address of the facility it was performed in;
- Nature of the abortion complication, diagnosis or treatment.
All signed and completed abortion reports must be sent to the State Department of Health and Senior Services within 45 days of the procedure. The Department must collect all abortion reports and is responsible for evaluating the data and publishing a statistical report annually.
