Health care providers must report diseases specified by the Commissioner of Public Health to the health director of the patient’s locality and the Department of Public Health (“department”) within 12 hours of diagnosing the disease. Providers may make such reports in writing, by telephone, or electronically. Local health directors and the department must ensure the confidentiality of the reports. Persons that violate this provision may receive a maximum fine of $500.
