Recordkeeping and Documentation Requirements
Providers participating in Medicaid must maintain records to fully disclose the nature, amount, and medical necessity of services provided to an eligible recipient. Providers that fail to keep adequate records will be found to be in violation of the Medicaid Providers Act. The provider records must detail the date, time, name of eligible recipient, provider, length of service, diagnosis, and medical necessity of the service. The provider record must also document any test results. A Medicaid provider must retain all medical records for at least 6 years from the date of payment.
