Reporting Requirements
The required financial and statistical report to be filed by licensed doctors must include information about the health care provider and/or clinic, including personnel and tax information. The report must also outline patient statements broken down by payer for Medicare, medical assistance/Medicaid, public payers, PPOs, non profit payers, commercial insurers, HMOs, worker’s compensation, car injury, patient payments (including copayments, deductibles), and other revenues. In addition, the report must identify contractual or capitated payments, a statement of operating revenue, a statement of expenses, a statement indicating time spent in completing the report, and a statement identifying any outside help used in preparing the report.
