Idaho Admin. Code r. 16.07.33.200, Individualized Treatment Plan

Requires the Department of Health and Welfare to create an individualized treatment plan for clients receiving adult mental health services. Requires a qualified physician to oversee the plan’s creation and implementation. Specifies the elements that the plan must address (e.g., necessary services, referrals to other services, post-discharge plans, etc.). Requires the client or their legal guardian to sign the plan as indication that they participated in its development and agree with its contents. Permits a plan to go unsigned if a provider documents the reason why the client or their legal guardian did not sign or refused to sign in the client’s record. Requires each plan to be reviewed and updated at least once every 120 days and for a new plan to be developed every 12 months.
