Idaho Admin. Code r. 16.03.10.524, Children’s DD HCBS State Plan Option: Plan of Service Process

Requires the Department of Health and Welfare to ensure that every participant who receives home and community based services (HCBS) has a plan of service that specifies the services they will receive. Requires the plan to be developed in coordination with the participant, the participant’s decision-making authority, and a Department representative and to be based on the results of a recent physical exam. Permits the plan developer to create the plan without the participant having attended a family-centered planning meeting, but requires the developer to justify the participant’s absence in the plan. Allows the participant’s decision-making authority to consent to having additional significant persons (e.g., family members) involved in creating the plan. Requires the plan to identify a plan monitor and specify the frequency of plan monitoring. Permits adjustments to the plan via an addendum. Requires reauthorization of services once per year.
