Requires providers of home and community based services to allow participants—or the participant’s decision-maker— to direct the creation of their service plan using a ‘person-centered planning process.’ Requires the planning process to occur at a time and place that is convenient to the participant, accounts for the participant’s culture, is accessible and in plain language, and include strategies for resolving conflicts during the process and avoiding conflicts of interest.
