Requires a physician to order home health services and specifies the information that physicians must include in these orders (e.g., the physician’s National Provider Identifier, the type and frequency of services, etc.). Requires physicians to reorder services that are necessary for extended periods of time at least once every 60 days or—for medical supplies and equipment— at least annually. Requires physicians to have a face to face encounter with the participant between 90 days prior to the start of home health services or 30 days after the start and to document this encounter in accordance with the CMS/Medicare DME coverage manual. Permits physicians to provide this encounter via telehealth or have a non-physician practitioner conduct the encounter so long as that practitioner reports the clinical findings to the physician.
