Permits Medicaid providers to receive Medicaid payment for sterilization procedures only if the participant receiving the procedure is at least 21 years old and has given their informed consent by voluntarily signing a Department issued consent form between 30 and 180 days prior to the sterilization procedure. Authorizes Medicaid payments for hysterectomies if the procedure is medically necessary, the participant’s desire to become incapable of reproducing is not the sole purpose of the procedure, the provider informs the participant that the procedure will render them unable to have children, and the participant signs the Department issued authorization form as an indication that they have received the requisite information.
