Requires licensed midwives to obtain and document a client’s informed consent. Specifies the information that the informed consent must address (e.g., the midwife’s experience, how to obtain the Board of Midwifery’s rules, how to file complaints, emergency protocols, etc.). Requires midwives to maintain a copy of signed informed consent forms for at least nine years following their last date of service.
