Defines the services that Healthy Connections participants who are enrolled with a primary care provider may receive (e.g., care management and coordination, a patient-centered decision making process, 24/7 access to an on-call provider, referral to other services, etc.). Specifies the times at which participants may select or change their primary care provider (e.g., upon becoming Medicaid eligibility, with cause as defined in the Medicaid Provider Handbook, etc.).
