Head and Spinal Cord Injury Reporting Requirements - Mo. Code Regs. Ann. tit. 19, § 30-41.010

All physicians and hospitals must report to the Department of Health registry any of the following:
- A head or spinal cord injury patient who is dead or dies upon arrival to an emergency department;
- Any head or spinal cord injury patient who is transferred to another hospital from the emergency department;
- Any inpatient admitted to the hospital to treat a head or spinal cord injury.
The registry form must include the following information:
- Hospital name and medical record number
- Patient name
- Social security number
- Date of birth
- Sex and race
- Previous treatment for the same injury
- If a minor (under 18), the above information for the parent or guardian is required
- Date and time on injury
- Cause of injury
- Scene of injury
- Hospital arrival information
- Hospital stay information
