Form for Reporting Abortions
Requires the State Office of Vital Records Services to create an abortion reporting form and specifies the information the form must include (e.g., age of pregnant person, type of facility that performed the abortion, etc.). Requires attending physicians to complete the form and submit it to the State Health Officer within 20 days of performing the abortion.
