Establishment of a Client Service Report System – N.Y. Mental Hyg. Law § 29.13

The commissioner must create a “client service report system” in order provide patients, patient representatives, and community and social services directors with information about patient services and treatments. Information distributed through this system must include “the types, location, date and estimated costs of services” as well as the names of service providers.
Patients may obtain their report upon request and may also request that their representative obtain a copy. Community and social services directors may obtain the reports so long as the patient and commissioner consent.
The commissioner must create procedures for “making such client service reports available.”
