This will open in a new window
Hospitals are required to submit to the Department of Health and Senior Services all acute care hospital encounters in an electronic format. The minimum data set that is to be reported must include elements such as record type, hospital identifier, type of encounter, patient medical record number, patient social security number, patient birth date, patient sex, race, ethnicity, address, and chief complaint.
The Department may release this information to a public health authority. However, if the public health authority re-releases the identifying information, prior authorization from the Department is needed. The Department may also authorize the public health authority’s contact with the health care provider or patient. The Department must notify hospitals of any noncompliance with these reporting obligations.
