All consumer information that is obtained due to an investigation into a health care facility for abuse or neglect is confidential. All unsubstantiated reports remain confidential following the conclusion of the investigation. Substantiated reports remain confidential to the extent the following information is included in the report: identifying factors of individuals, personnel records, dates of birth, and social security numbers, among other information.
