Providers must report any complaints about abuse or neglect on the Department’s designated incident report form. The form is available on the Department’s website, by telephone request, or by facsimile request. The reported information must contain details about the alleged abuse or neglect; the name, age, and gender of the patient and the individual suspected of abusing or neglecting the patient; and the condition of the patient, among other requirements.
