Clinical Records
Home Health Agencies must maintain clinical records for each New Jersey Medicaid/ NJ FamilyCare beneficiary. The records must include the beneficiary’s identifying information, the § 10:60-2.3 care plan, contact information for the beneficiary’s physician, signed and dated clinical notes, summary reports, and transfer information. The Home Health Agency must submit summary reports to the attending physician every two months, at minimum. Home health Agencies must transfer beneficiaries to alternative health care providers when necessary.
