Plan of Care
An attending physician must develop a New Jersey Medicaid/ NJ FamilyCare beneficiary’s plan of care for home health services in cooperation with the home health agency that will provide the services. Home health services care plans must address the beneficiary’s medical, nursing, and social care needs and must be reevaluated and revised every two months. Specific elements of the care plan include the beneficiary’s case history, the type and amount of home health services that will be provided, treatment goals, and discharge planning.
