Equity and Disparities in Minnesota
The state of Minnesota has addressed health disparities and barriers to care by implementing patient centered health care homes. The health care home model emphasizes appropriate care based on the patient’s race, ethnicity and primary language.1 An annual evaluation of the health care home must take place, which includes the number of state health care beneficiaries enrolled in health care homes, key characteristics of enrollees, including number and type of chronic conditions, and race, ethnicity and language.2 Minnesota law also authorizes assessment of whether the Medicaid and community health center programs adequately address health disparities and access to care by racial and ethnic minorities within the state.3
