Provider Responsibilities for the Trauma Registry

The Department of Health maintains a trauma registry, the purpose of which is to provide data for injury surveillance, analysis, and prevention programs; monitor and evaluate the outcome of care of major trauma patients; assess compliance with state standards for trauma care; provide information for resource planning, system design and management; and provide a resource for research and education.

All trauma care providers must protect the confidentiality of data in their possession and as it is transferred to the department.  For pre-hospital services that transport trauma patients, the service must provide an initial report of patient care to the receiving facility at the time the trauma patient is delivered, and within ten days after the trauma patient is delivered, send a complete patient care report to the receiving facility to include data listed in the regulation.  For designated trauma services, each service must have a person identified as responsible for coordination of trauma registry activities, report certain data elements as listed in the regulation, and report patients in a calendar quarter in a department-approved format by the end of the following quarter.  Finally, for designated trauma rehabilitation services, the services must provide data to the trauma registry as requested by the Department.
