Cancer Registry Reporting Requirements
The responsibility for identifying and reporting cases of cancer rests with health care facilities, independent clinical laboratories, and other principal health care providers. Cancer diagnosis and treatment facilities that diagnose an incidence of cancer must submit case finding documents by paper form, computer disk, or electronic file or make batched hard copy documents available for on-site review, within forty-five days of the date of service. The case finding documents should include disease and operation indices for cancer cases, pathology and cytology reports, new patient radiation logs, new patient chemotherapy logs, and other alternative case finding documents that are necessary to identify or verify reportable cancer cases.
Principal health care providers must, on request by the Department of Health or its designee, identify reportable cancer cases diagnosed at facilities other than hospitals, surgical centers, and outpatient radiation therapy centers, unless the patient was hospitalized for additional cancer diagnosis or treatment services within one month of diagnosis.
