Voluntary Disclosures
Permits individuals or their authorized representatives to consent to the disclosure of their mental health information by signing a written authorization. Specifies the information that mental health providers must include on this authorization (e.g., duration of the authorization’s validity, nature of information disclosed, etc.). Requires providers to give the authorization to the individual or their representative and to place a copy in the individual’s mental health record.
