Prescription information submitted to the Arizona state board of pharmacy as part of the controlled substances prescription monitoring program is confidential. The board may disclose prescription information to licensing boards or law enforcement agencies if the board suspects unprofessional or illegal conduct. The board may also release information from the program to providers that prescribe or dispense controlled substances, the Arizona health care cost containment system administration, persons that have a valid court order, medical examiners that need the information in relation to a death investigation, and individuals that wish to access their own prescription information.  Arizona permits the board to release de-identified data for research purposes.
