“Subpoenas; certification requirements”
Arizona permits the use of a subpoena to access individually identifiable health information maintained by a health information organization if the party complies with A.R.S. § 12-2294.01 (regarding the release of medical or payment records pursuant to a subpoena) and a court determines that the information cannot be obtained from the original source and is either relevant to the proceeding or may lead to the discovery of admissible evidence.
