Statewide registry
Requires the Department of Public Health to establish a statewide immunization and health screening registry that will contain screening data and information regarding screening follow-up. Specifies the information that must be reported to the registry (e.g., patient name, vaccine received, date of vaccinations etc.). Classifies registry information as confidential, but permits disclosure in limited circumstances (e.g, to the patient or their guardian, to schools for admission purposes, etc.).
