An individual may revoke a declaration relating to the use of life sustaining procedures “at any time and in any manner by which [they are] able to communicate an intent to revoke…” An attending physician or health care provider is only subject to the revocation if the revoking individual or a person that witnesses the revocation communicates the revocation to the physician or provider.
An attending physician or health care provider must document the revocation of a declaration relating to the use of life sustaining procedures in the revoking individual’s medical record.
