Hospitals, ambulatory surgery centers, nursing homes, or other health care entities must any disciplinary action taken against a health care provider or voluntary resignation of a health care professional to the appropriate licensing body. All reports must be submitted within 15 days of the action, and must include the following:
- Name, address, and phone number of the person making the report;
- Name, address, and phone number of the person who is the subject of the report;
- A description of the facts that gave rise to the report;
- Information on potential litigation, if court action is involved.
The health care entity or the licensing authority may not be liable for reporting any of the information required under the law.
