Reports to Department -- immunity -- confidentiality -- investigations
Requires health care providers and facilities, upon treating a person with a reportable disease, to report the incidence to the Department of Public Health and, when applicable, to the local health department. Specifies the information that must be included in the report (e.g., patient name, patient contact information, test results, etc.). Classifies reported information as confidential. Grants the Department authority to subpoena records and information necessary to investigate the report.
