The local provider of protective services must transmit to the department of aging written reports involving sexual abuse, serious physical injury, serious bodily injury or suspicious death of a recipient of services from any of the following facilities:
- Domiciliary care home;
- Home health care agency;
- Long-term care nursing facility;
- Older adult daily living center; or
- A personal care home.
Reports must include the following information:
- The name and address of the alleged victim;
- Where the suspected abuse occurred;
- The age and sex of the alleged perpetrator and victim;
- The nature and extent of the suspected abuse, including any evidence of prior abuse;
- The name and relationship of the individual responsible for causing the alleged abuse to the victim, if known, and any evidence of prior abuse by that individual;
- The source of the report;
- The individual making the report and where that individual can be reached;
- The actions taken by the reporting source; and
- Any other information which the department may require by regulation.
