Physicians must submit a report to the director of health within 48 hours of attending on or being called in to visit a patient whom the physician believes to be suffering from any of the following occupational diseases or occupational-related ailments:
- Poisoning from lead, cadmium, phosphorous, brass, arsenic, mercury, wood alcohol or their compounds; and
- Compressed air illness.
The report must contain the following information:
- The name, address, and occupation of the patient;
- The name, address, and business of the patient’s employer;
- The nature of the disease or ailment; and
- Name, address, and telephone number of the physician.
